Civil Surgeon
Dr.
Augusto Focil is certified as an immigration civil surgeon to
provide immigration physicals to immigrants who are seeking
adjustment of status.
We
understand the cumbersome process and the expenses the patient
incurs when the physical is provided. We have streamlined this
process by providing fast service at an economical price. We
welcome same day appointments for those who are in dire need to
have the physical, vaccinations, and lab work done in a moments
notice.
For
patients who need exceptional service because of time
constraints, an expedited service is available at an additional
price. We strive to be the best in this area and work closely
with other doctors, and labs whose reports diagnosis depends on
the pace of the physical evaluation.
Patient information:
Dr. Augusto Focil is an USCIS civil surgeon
certified to perform immigration physicals. Please read
carefully and follow the instructions listed below.
- Prior to your immigration
date the patient should schedule an appointment for a physical
examination at least 10 days before.
- The cost of a physical
exam, laboratory exams, and tuberculosis exam is $149. PLEASE
NOTE THIS DOES NOT INCLUDE ANY VACCINES OR X-RAYS.
- Dr. Focil will need at
least 5 working days to process any exam results given by the
laboratory. Patients with abnormal results will need to have
other exams done; this will delay the process 4-6 business days.
- After 48 hours of the
initial tuberculosis exam the patient will need to come back to
see Dr. Focil to review the tuberculosis results. IT IS VERY
IMPORTANT THAT THE PATIENT FOLLOWS THESE INSTRUCTIONS.
- In case that the results
are positive the patient will need to have a chest X-ray, which
is $89.00. The patient has the option to have the x-rays done at
another facility of their choice.*
- All vaccines can be done
within our facility or can be done at another facility of the
patient’s choice.
- All medical records and
vaccination documentation should be brought to Dr. Focil on the
first office visit. If the patient brings the medical records
and vaccines after the first visit, their will be a $60 medical
records review charge.
- All physical examinations
are valid up to one year from the date of exam and all
laboratory exams are only valid for one month.
- It is the responsibility of
the patient to call FocilMed, Inc a day before to schedule a
pick up date. This gives us time to make sure all immigration
forms have been properly filled. It is also the patient’s
responsibility to pick up all completed immigration forms at
FocilMed, Inc. THE PATIENT UNDER NO CIRCUMSTANCES SHALL OPEN THE
SEALED ENVELOPE PROVIDED BY DR. FOCIL. THIS IS TO BE DELIVERED
DIRECTLY TO THE USCIS OFFICE OR TO YOUR ATTORNEY.
- IF FOR ANY CIRCUMSTANCES
THE SEALED ENVELOPE IS OPENED, LOST, OR STOLEN THERE WILL BE A
$60 SERVICE FEE. This fee includes all necessary paper work,
materials, and the time it takes for Dr. Focil to review your
medical records.
- Any patient that is rude,
physically or verbally abusive will not be permitted to continue
his physical with Dr. Focil. Any form of abnormal behavior will
not be tolerated and we do reserve the right to refuse service.
NO REFUNDS WILL BE GIVEN.
- Pick up schedules for
immigration forms are the following: M-F 9:00am-6:00pm* (2pm-3pm
are closed for lunch) and Sat 9:00am-2:00pm.
